Position: Social Worker – AYP

Daraja Mbili Vision Volunteers is a Community-Based Organization, registered in Kenya as a non-profit organization. As an organization, we envision to work towards having a Heathy and Empowered Society. DMVV’s mission is to enhance sustainable health access to communities in Kenya through implementation of health and social development programs that contributes to health promotion, diseases prevention, mitigation of suffering and empowerment for all. DMVV is implementing Global Fund HIV grants through Kenya Red Cross Society to implement comprehensive HIV prevention, care and support program for AYP and PLHIV in Kisii County. The program is seeking to fill the following positions in Kisii County.

Successful candidates will be expected to utilize a targeted approach to youth development by providing safe spaces for adolescents and young people to access information on sexual and reproductive health services, HIV/AIDS awareness, information resource center and recreational activities that are critical to youth-led development.

Job Summary
Reporting to the site in charge, the social worker will be responsible for the overall planning and providing individual or group psychosocial support. Develop and monitor a case management system for monitoring of referred cases. Work with individuals and groups to improve mental health.

Responsibilities and Tasks
• Conduct psychosocial support groups on a monthly basis in collaboration with other YWC team.
• Follow up and ensure all enrolled HIV positive clients are followed and Viral Load results obtained from the linked facilities.
• Participate and ensure community mobilization is done and program targets in viral load acquisition is achieved 100%.
• Follow up and ensure clients on ART who are LTFU are returned to care and treatment.
• Assist in the community ART distribution to the clients on DSD model.
• Establish therapeutic rapport with AYP with a view to offer individualized and group counselling within the YWC and
during outreach activities.
• Sensitize, problem solve, dialogue and mediate with the AYP about issues of trauma, GBV any other related psychosocial issues
• Plan and conduct support group meetings for AYLHIV.
• Maintain and update referral directory for Linkage.
• In collaboration with the other healthcare providers in the YW, the Social Worker will implement AYP EBIs e.g. Respect K, SHUGA, MHMC and any other nationally approved EBI.
• Establish course of action by exploring options; setting goals and following up with AYP on safe sexual practices.
• Supports service completion for the AYP by referring him/her to other community resources, arranging for appointments, and establishing rapport with other agencies.
• Strengthen the existing referral systems with other stakeholders.
• Maintains record of case by documenting client’s situation and client’s own actions.
• Maintains operations by following policies and procedures; participating in quality reviews; reporting needed changes.
• Maintains client confidence and protects ethical considerations by keeping information confidential.
• Contributes to team effort by accomplishing related results as needed
• To submit a monthly statistical and narrative report as required by the program supervisor within agreed timelines for timely follow-up and decision making.

Minimum Qualification: –
Education
• Diploma in Community Health and Development or equivalent
• Relevant experience as an Adolescent and Youth Program facilitator (EBI or Mentorship intervention) or in similar position.
• HTS/adherence Certificate
• Understands the HIV-related needs of AYP, especially AGYWs and have worked to support and implement activities to support access to SRH services, stigma reduction and discrimination.
• Good interpersonal and communication skills, ability to work independently with minimal supervision and over long hours.
• Good oral and written communication skills in Kiswahili and English,
• Computer literacy
• Ability to generate quality reports.
• Excellent organizational and analytical skills.
• Ability to thrive in a fast-paced, multi-tasking environment,
• Able to work in challenging field conditions and within tight timelines.
• Knowledge of local area and community structures desirable.
